If you would like to mail in your check payment registration, please complete this online registration and select check as payment, then mail your check (made payable to CME Science) to the address below with a print out of your registration. If you would like to fax in your credit card payment registration, please comeplete this online form, print and fax to 1-650-887-2113.

Refund Policy: To be eligible for a refund less a $100 administrative fee, cancellations must be made within 24 hours of purchase Please note: If CME credit has been applied for during the 24 hour period the purchase is not eligible for a refund.
